If it's a question, the subject of which you're not sure about, ask a clarifying question as a follow up. By this do you mean, such and such, or are you referring to something else. Clarifying the point. If the issue is the person speaks in a cadence, or has an accent that you're having trouble understanding, you do much the same thing. "I just want to make sure I understand the question correctly. The way I heard it was such and such, is that what you were saying?" You're not saying I don't understand what you're saying.
